Appointments

For all appointments and enquiries please ring 02 9621 2133 during the Blacktown Office Hours 8.30am – 5.00pm (Monday-Friday).

CONSULTATION CHECK LIST

• Make sure you have booked and recorded the correct date, time and place for your consultation.
• Arrive 10 minutes prior to your appointment.
• Bring your referral letter if given to you by your doctor.
• Bring hard copies of your radiographs and CT scans if available, or the details of the date, when and where you had your films taken.
• Complete your “Health Questionnaire” online and either bring a copy of it with you or submit it online.
• Bring a list of all your medications that you take (tablets, capsules, creams, injections etc) if you have not already listed them in your health questionnaire.
• If you have a complex medical history preferably bring a “Health Summary” from your Medical Doctor.
• Bring your Medicare card.
• Children under 16 years of age need to be escorted by a responsible adult, preferably their parent.
• Bring an interpreter if you are unable to speak and understand English satisfactorily.
• If you or your escort have had COVID-19 within the last 7 days or are unwell on the day of your consultation, please phone 02 9621 2133 to reschedule your appointment.
