For Referrers

Referral Details

• Complete the “referral form” template (or on the provided referral pad). Ring the Blacktown rooms if you require further pads.
• Include any significant past or present medical history, allergies, medications and any pertinent histopathology and biochemistry results relating to your referral.
• Attach any relevant radiographs and CBCT scans.
• If your patient has been prescribed any of the following classes of drugs -monoclonal antibodies, bisphosphonates or immunosuppressive drugs, please advise commencement date and last date given.
• If you prefer to use your own letterhead, kindly email the referral and the relevant scans/radiographs to our office
• If your patient does not have hard copies of their radiographs and scans, please indicate where and when they had them taken.
